Skilled Nursing Visits

To ensure that our end-of-life patients can live their remaining days in comfort and dignity, Carelife Hospice LLC can provide visiting hospice nurses to their homes or any other care setting of their preference. Our hospice nurses can perform a wide range of medical and non-medical care services.

Bereavement Support

No one is truly prepared for the passing of a loved one. During this difficult time, know that there is someone who can help you overcome the feelings of anger, sorrow, pain, and helplessness after a loss. We have a team of social workers, counselors, and trained volunteers who can provide you with the assistance and resources you need.

At Carelife Hospice LLC, we know that it takes a team to provide the best hospice care possible. We coordinate everything with healthcare providers, social workers, trained volunteers, and other members of your care team so you can focus on spending quality time with your loved ones.

Pain Management

Carelife Hospice LLC knows that to ensure the quality of life for individuals who are at the end of life, they must be free from any discomfort brought about by their life-limiting condition. With the help of our interdisciplinary team, we will develop a care plan that will minimize pain as much as possible.

Attending Physician

  • Co-certifies the terminal prognosis
  • Assesses patient needs, manages symptoms, prescribes treatment
  • Directs and approves the plan of care
  • Coordinates care with the interdisciplinary team

  • Assesses patient/family emotional, social and financial needs
  • Develops plan of care to meet identified needs
  • Provides direct counseling
  • Refers patients/families to appropriate community services

Chaplain

  • Assesses patient/family for spiritual needs
  • Develops plan of care to meet identified needs
  • Provides bereavement support

Volunteers

  • Provides companionship, support to patient/family
  • Provides needed non-medical services
  • Provides support at time of death and during bereavement
